Findings
The facility failed to follow Alleged Victim's (AV) care plan requiring a modified diet. On or about July 27, 2022, AV was served a regular meal and AV began choking, requiring AV to go to the hospital to clear the dislodged food. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
