Findings
Alleged Victim (AV) is care planed for total assist with bathing and two baths a week. Documentation indicates on two (2) occasions AV has gone seven (7) days without a shower, and one (1) time went twenty-two (22) days without a shower. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
