Findings
The facility failed to implement interventions and provide appropriate services related to the Alleged Victim’s (AV) personal hygiene care needs and resistance to that care which resulted in AV's care needs not being met resulting in personal loss of dignity. Facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
