Findings
The facility failed to implement interventions or appropriately care plan for Witness #1’s (W1) known behaviors. On or about July 31, 2022, W1 inappropriately touched the Alleged Victim (AV), placing AV at risk for emotional harm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
