Findings
Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) has history of aggressive behaviors. On or about August 03,2022, Witness 1 (W1) struck AV with pillows, and struck AV on the right side of h/h head leaving a lump. The facility failed to adjust the service plan for W1 leading to continued aggressive behavior. Facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
