Findings
Based on an investigation, it was determined that on or about August 3, 2022, Alleged Perpetrator 2 (AP2) handled the Alleged Victim (AV) in a rough manner during a change of brief and clothing, which caused AV emotional distress. AP2s actions are considered physical abuse. The facility failed to protect a resident from physical abuse which violates Oregon Administrative Rules.
