- On or about August 10, 2022, Alleged Perpetrator 2 (AP2) was verbally aggressive towards the Alleged Victim (AV), which caused AV emotional distress and loss of personal dignity. AP2's actions are considered neglect of care and constitute abuse. The facility failed provide a safe environment which is a violation of Oregon Administrative Rules.
