Findings
The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs, relating to falls. AV experienced approximately 8 falls from approximately June 24, 2022, and August 10, 2022, which resulted in repeated unreasonable discomfort as a result of a head injury and some skin tears. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
