Findings
The facility failed to appropriately care plan for Alleged Victim (AV) and Witness #1 (W1) regarding their known behaviors and ongoing issues towards each other. AV was visibly upset regarding W1 and their interactions with each other that cause emotional distress. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
