Alleged Victim (AV) Slid off the bed on or about May 01, 2022, sustaining skin tear, bruise, and rib fracture. AV Slid off the bed on or about May 17, 2022. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s sliding of the bed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
