The facility failed to implement interventions and appropriately monitor Witness 1 (W1) after a resident-to-resident altercation with Alleged Victim (AV) which resulted in a second, same day altercation between W1 and Alleged Victim (AV), causing injuries to AV's face and hand which is a violation of resident rights, is considered neglect of care and constitutes abuse.
