Findings
On or about August 11, 2022, Witness #3 (W3) was training Alleged Perpetrator #2 (AP2) on medication administration. AP2 was not supposed to be dispensing medications, rather watching W3 to learn the position. AP2 administered medication to the Alleged Victim (AV) that was meant for another resident. AV had a reaction to the medication where his/her heart rate dropped and was unconscious. AV was transported to the hospital for evaluation. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
