Findings
On or about August 11, 2022, the Alleged Victim (AV) was found to have a bruise on his/her right and left wrist. It is unknown how AV sustained this bruise. AV has a history of bruises of unknown cause and fragile skin. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
