Findings
On or about August 11, 2022, the Alleged Victim (AV) was watching a movie and refused to have his/her incontinence briefs changed by staff. AV's care plan does not direct staff to re-approach AV or any time line in which AV should be changed. At approximately 10:00 pm, AV was ready for bed and when staff went to change AV, it was discovered that AV's brief had not been changed since 1:30 pm earlier that day. AV was in the same brief for over 8 hours, which was extremely soiled, causing redness and unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
