Findings
The facility, and Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system to ensure the Alleged Victim's (AV) medication was administered as ordered. On or about August 14, 2022, AV did not receive h/h narcotic pain medication as prescribed, resulting in AV being upset and not feeling well. The facility, and AP2’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
