Findings
Witness #1 (W1) is a known wanderer and is known to get into altercations with other residents. On or about August 17, 2022, W1 wandered into the Alleged Victim's (AV) room and hit AV in the face. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
