Findings
On or about August 23, 2022, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) did not follow proper medication procedure and AV was given the wrong medication, exposing him/her to risk of serious harm. AP2's and AP3's actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment, which violates Oregon Administrative Rules.
