Findings: The facility failed to provide a safe environment. According to an investigation, the Alleged Victim (AV) and Witness 1 (W1) have history of behaviors and resident-to-resident altercations. On or about August 23, 2023, W1 pushed AV down, resulting in a scratch to AV’s back. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
