- On or about December 4, 2020, the Alleged Perpetrator 2 (AP2) had been counseled by facility management after they were made aware that AP2 was spending an inappropriate amount of time with the Alleged Victim (AV). AP2 denied any inappropriate contact at that time. On or about March 26, 2021, AP2 was found in AV’s room with AV not having any pants on, AP2 and AV denied inappropriate contact again, however, AP2 was counseled and advised that contact with AV must be in public areas. AV moved from the facility and contact with AP2 continued. On or about December 2021, AV accused AP2 of rape. AP2 and AV admitted to having a sexual relationship. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes sexual abuse. The facility failure is a violation of Oregon Administrative Rules.
