Findings
On or about August 25, 2022, Alleged Perpetrator 2 (AP2) made an inappropriate verbal comment toward the Alleged Victim (AV) in his/her presence. AP2 is responsible for verbal abuse. The facility failed to provide AV from inappropriate verbal comments made by staff and the failure is a violation of Oregon Administrative Rules. Alleged Perpetrator 2 (AP2), Alleged Perpetrator 3 (AP3), and Alleged Perpetrator 4 (AP4) allegedly neglected AV. An investigation determined no abuse/wrongdoing occurred.
