- Alleged Victim (AV) is a high fall risk and has history of falls. AV is care planned as independent with ambulation, using an assistive device. On or about August 23, 2022, and August 26, 2022, AV fell resulting in h/h being sent to the Emergency room (ER) due to swollen, painful knees and having difficulty standing and transferring. AV returned from the ER with a diagnosis of joint effusion and wearing a knee brace immobilizer. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
