Findings
The Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim (AV) care plan. AP2 failed to ensure the AV's fall prevention devices were in place before leaving the AV alone, resulting in a fall with head injury to the AV. This failure by AP2 is considered neglect. The facility failed to protect AV from neglect, which is a violation of Oregon Administrative Rules.
