Findings
On or about August 25, 2022, Alleged Perpetrator #2 (AP2) was giving the Alleged Victim (AV) a shower. AP2 rinsed AV off with cold water, causing unreasonable discomfort to AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
