Findings
On or about September 3, 2022, Alleged Perpetrator #2 (AP2) administered the incorrect medication to the Alleged Victim (AV). AV did not have any negative outcome, however, AV was placed at risk for harm. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility failure is a violation of Oregon Administrative Rules.
