Findings
The facility failed to provide appropriate services according to the Alleged Victim's personal hygiene care needs which resulted in AV having long toe nails and crusting around his/her eyes causing unreasonable discomfort and resulting in personal loss of dignity. The facility's failure is a violation of resident rights, is considered in neglect of care and constitutes abuse.
