Findings
The facility failed to ensure supervision and staff support regarding known behaviors of Witness #1 (W1). W1 has multiple behaviors that upset the Alleged Victim (AV), and AV perceived W1's actions as a threat to his/her safety. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
