Findings
The facility failed to follow the care plan. Alleged Victim (AV) service plan indicates to use mechanical lift for all transfers. Two person Hoyer assist for all transfers. On or about September 11, 2022, agency staff attempted to transfer AV out of bed by stand by assist, resulting in AV slipping from the bed to the floor. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
