Findings
On or about September 12, 2022, the Alleged Victim (AV) was found to have a bruise on his/her head and right index finger. AV's injuries were the result of an unwitnessed fall. The facility failed to provide a safe environment, which was a violation of resident rights, is considered neglect of care and constitutes abuse.
