Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)needs, the facility failed to appropriately care plan to ensure alert pendant was placed on AV after bathing and dressing. On or about September 19, 2022, AV woke in the night and could not find h/h call pendant for assistance resulting in AV getting out of bed and sustaining bruises from bumping legs on side of bed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegations that Alleged Perpetrator #2 (AP2) failed to provide service was investigated, and wrongdoing was not substantiated.
