- The facility failed to follow the Alleged Victim (AV) care plan. AV has history of wandering into other residents’ rooms. AV care plan indicates if AV begins to wander to close to Witness 1 (W1) room, staff to promptly redirect AV away from W1. On or about, September 22, 2022, AV wandered into W1 room, and was slapped in the face by W1. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
