Findings
Alleged Victim (AV) had a known elopement risk and had a history of wandering and exit seeking. On or about September 29, 2022, AV was discovered to be missing and found at a grocery store. The facility failed to provide a safe environment for AV which is a violation of resident rights, is neglect of care which constitutes abuse.
