Findings
The Alleged Victim (AV) is care planned for stand-by assist with all ambulation/mobility needs and has a history of falls and self-transferring without assistance. The facility failed to properly care plan to mitigate risk of falls and self-transferring. AV had approximately six unwitnessed falls between June 30, 2022, and September 6, 2022, some with injuries. On or about September 29, 2022, AV had another fall while using the restroom without assistance, which resulted AV experiencing pain. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
