The facility failed to provide appropriate services according to the Alleged Victim's (AV) needs. The facility's failure caused AV to get a bruise the size of a silver dollar on his/her left hand, causing AV unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
