Findings
Witness #1 (W1) is known to have altercations and between August 1, 2022 and October 2, 2022, W1 was physically aggressive towards others at least 16 times. On or about October 2, 2022, W1 and the Alleged Victim (AV) were caught hitting each other in the dining room. AV was not injured, however, he/she was placed at risk for harm. The facility's failure to properly care plan to address W1's physical behavior is a violation of resident rights, is considered neglect of care and constitutes abuse.
