Findings
According to documentation Alleged Perpetrator 2 (AP) failed to remove themselves from an altercation with the Alleged Victim (AV). The failure resulted in AP2 grabbing AV’s wrists and hands and pushing AV, causing bruising and emotional discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
