Findings
The Alleged Victim (AV) receives inject-able blood sugar medication when his/her blood sugar count is above 110. On or about October 3, 2022, Alleged Perpetrator #2 (AP2) gave AV his/her medication even though his/her blood sugar was below 110. AP2's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failure is a violation of Oregon Administrative Rules.
