Findings
On or about October 11, 2022, the Alleged Victim fell and fractured his/her hip. AV had 5 previous falls in 2 months. The facility failed to properly care plan and implement interventions to ensure AV's safety. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
