Findings
According to documents, the facility failed to follow the care plan for the Alleged Victim (AV) to put on non-skid socks and have a fall matt in place to prevent future falls. The failure resulted in the AV falling and hitting their head causing a laceration, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
