Findings
According to documentation, AP2 failed to follow AV's behavioral support plan when s/he shows signs of aggression. The failure resulted in AP2 spitting back at AV while s/he was exhibiting signs of behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
