Findings
On or about October 21, 2022 at approximately 1:00 am, the Alleged Victim (AV) was found on the floor in his/her room with an injury from a fall. AV has had 6 falls recently. The facility failed to implement interventions to prevent AV from falling, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
