Findings
The Alleged Victim (AV) was care planned for a calm and quiet environment. Based on an investigation, the Alleged Perpetrator (AP2) did not follow AV's care plan when AP2 caused a loud and chaotic environment for AV, which resulted in AV experiencing unreasonable emotional discomfort. AP2's actions are considered neglect and constitutes abuse. The facility failed to ensure care plans were followed which is a violation of Oregon Administrative Rules.
