Findings
On or about October 27. 2022, the Alleged Victim (AV) was found on the floor beside his/her wheelchair. AV complained of pain and was sent out to the emergency department for evaluation. AV returned the next day and was found to have fractured ribs. The facility failed to properly care plan to keep AV from slipping off his/her wheelchair, which is a violation of Oregon Administrative Rules.
