Findings
According to the documentation, the Alleged Perpetrator (AP2) failed to follow the care plan when assisting the Alleged Victim (AV) with hygiene care. The failure resulted in the AP2 grabbing the AV by the inner thigh to turn them in bed causing the AV to yell out in pain and left red marks on their thigh, which is a violation of resident rights and is considered physical abuse. The facility failed to follow the care plan which is a violation of Oregon Administrative Rules.
