Findings
On or about October 28, 2022, Alleged Victim (AV) was found to have a quarter-sized bruise on his/her forehead. Based on interviews and facility documentation, AV had four (4) bruises within the two (2) months prior to the incident without documented interventions to prevent the bruising. The facility failed to protect Alleged Victim (AV) from continued bruising, causing AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
