Findings
The Alleged Victim (AV) is care planned for physical assistance with all ambulation, mobility, and transfer needs. AV has a history of falls and self-transferring without assistance. According to the investigation, the facility failed to properly care plan to mitigate risk of falls and self-transferring. AV had approximately four falls between June 17, 2022, and October 23, 2022, some with injuries. On or about October 27, 2022, AV suffered a non-witnessed fall and was found on the floor by AV’s bed, which resulted in AV experiencing pain. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
