Findings
On or about October 22, 2022, AV was yelling for staff to help him/her. Staff responded and it was discovered that an unknown Alleged Perpetrator #2 (AP2) stuffed toilet paper into AVs call light, making it inoperable and AV was unable to pull it to request assistance. AP2’s actions are a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure a safe environment, which is a failure of Oregon Administrative Rules.
