Findings
Alleged Victim (AV) is a high fall risk and is care planned for an escort while out of the community. On or about November 04, 2022, AV fell while walking in the park without an escort. AV had pain and swelling to the right lower rib cage and was transferred to the hospital. AV was diagnosed with displaced and broken ribs. The facility failed to follow AVs care plan, which is a violation of resident’s rights is neglect of care and constitutes abuse.
