Findings
The Alleged Victim (AV) is a know fall risk. Between July 3, 2022, and July 28, 2022, AV fell out of his/her wheelchair 4 times. After the 3rd fall, the facility ordered a pad for the wheelchair. There were no other interventions to prevent falls put into place after the falls on July 3, when AV suffered a bruise, July 23rd or on July 28th when AV suffered a bloody nose and a skin tear. The facility's failure to properly care plan for AV's falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
