Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, number of falls with lack of appropriate interventions. Based on the investigation, AV experienced approximately four falls, between October 24, 2022, and November 11, 2022. On or about November 12, 2022, AV experienced another fall, which resulted in AV receiving eleven staples on his/her right knee. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
