Findings
The facility failed to follow the Alleged Victim's (AV) care plan and provide appropriate supervision according to his/her needs and the wandering behavior of others. The failure resulted in AV being injured another resident and him/her room being destroyed without staff intervention or knowledge, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
